Review of Paranoid Reaction ana proposal of working principles in therapeutic management of Infidelic Paranoia.


Abstract
In spite of the problems of Paranoia, especially infidelic Paranoia have been most painful, dangerous and poisonous threatning to the marrital life and family, but in most cases psychiatrists have been helpless, frustrated and confused in dealing with the problems.
The purpose of the study is to review various former writer¢¥s views on Paranoia and our clinical experiences with Paranoia, and to propose our operational¢¥ working principles being practiced for the infidelic paranoia with those who have intense, overwhelming and prominent delusion of infidelity in the Psychiatric Department of Korea General Hospital.
We have presented several cases of Paranoia and have reviewed psychodynamic views of various writer¢¥s like; Kreapelin, Freud, Mayer, Bleuler, Kretschmer, Knight, Numberg, Klein, Glover, Cameron, Waedler.,
It is learned that the most prevalent and commonly accepted views of Psychodynamics are characterized by the mechanisms of fixation, regression, denial, projection, masochism and sadism.
The effort is made to evaluate psychopathological dynamics of the cases from the theoretical view points as well as the actual clinical experiences we have had, and the most impressive factors are thought to be how to see and perceive their parental roles, and how and what to be taught in perceiving the world, person, masculinity and femininity, and the model of courtship and royalty of their parents in their marrital relationship.
The mode of paranoid thinking was diffuse, vague, floating, subtle and not well circumscribed in the early stage, however as it progress becomes focussed, crystalized and concretized organizing of the world¡þparanoid pseudocommunity.
We are also quite impressed with the nature of precipitating factors which are closely related with the rapid development of industry in the confucianism oriented Korean ethical society.
The cases we have experienced give us strong implication that the male infidelic paranoia tends to very
likely identify his wife with the female object with whom he had promiscuous extramarrital sexual affair, and the female infidelic paranoia who bad longing for untreated sexual fantasy onto her husband.
We also discussed some special points of difficulties in treatment and management of the cases of infidelic paranoia who belongs to the upper and highly sophiscated social group.
Finally, we propose one of the operational working principle which we have been practiced for years,
and considered to be effective and promising in handling the cases of infidelic paranoia;
1). "Open-let-Know" principle in which the both side family are told the matter of fact in details as much as possible: By doing this theoretically simple but practically energy-consuming work, the victims get more and better understanding and emotienal supports from the surrounding in one side. and in the other side the patient¢¥s delusions are diluted or desensitized in their intensity and at the same time more adequate breaks are given to the patient¢¥s delusional acting out.
2). The problem of divorce of infidelic paranoia which therapist would frequently face should also be discussed openly among the family, and in most cases this procedure can be carried out without so much pain imposed when the "open-let-know" principle is worked out successfully.
3). By passing through the steps of operational working principle, we often found the patient got better motivated to accept the responsibility in the planning of children¢¥s emotional hygine and the outcomes were less tragic.
Actually, by applying these principles the infidelic paranoia couples and family were protected and prevented from tragic break-down in considerable number of cases, and it should also be mentioned that the coming; secone I and third victims can be prevented from the ever-lasting infidelic paranoia¢¥s serching for target.
It is worthwhile to mention that infidelic paranoia is considerebly prevalent than expected among the active homosexual couples in our experienses, but the statistical data will be followed on next study.
